The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
We are seeking a passionate and experienced Community Health Care Registered Nurse to join our team at Hawkesbury District Hospital. This role provides high quality nursing care to individuals, carers and families within the community, with the aim of maintaining and/or improving health, independence and wellbeing.
The Community Health Care Registered Nurse delivers person centered care guided by Primary Health Care principles, evidence based best practice, and the Australian Aged Care Quality Standards. Care is provided through a flexible, responsive wellness and reablement approach, in partnership with clients, families/carers, General Practitioners and other key stakeholders.

What you will bring to the role
- Current registration as Registered Nurse Division 1 with AHPRA.
- Demonstrated recent experience and clinical skill in general nursing as a registered nurse (including completion of a new graduate RN program or 12 months full time equivalent experience working as a Registered Nurse).
- Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
- Demonstrated evidence of being able to provide person centred care and an ability to liaise with other health professionals to facilitate case management and care planning.
- Demonstrated effective communication skills (written and verbal) and interpersonal skills.
- Demonstrated commitment to professional development, evidence based practice and an awareness of changes within the health care environment.
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
- Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
