About the Role
Working within a student services and transition-focused team, this role provides targeted, case-managed support to young people during a critical period of change. The successful applicant will apply a strong Deaf cultural lens, fluency in Auslan, and an understanding of youth mental health and wellbeing to ensure students' communication, access and wellbeing needs are effectively supported.
You will work collaboratively with schools, families, employers, training providers and community agencies to develop inclusive, culturally safe and practical transition pathways that improve post-school outcomes for DHH students.
Key responsibilities include:
- Providing targeted transition support and case management for Deaf and Hard of Hearing students.
- Supporting schools to interpret, develop, refine, and enact departmental policies, practices, and procedures relating to students exiting school prior to the completion of Year 12.
- Developing and implementing effective retention and transition strategies and programs to improve outcomes for at-risk students.
- Identifying student needs, options, and interventions to assist young people and their families in transitioning into viable education, training, or employment pathways.
- Collating and analysing confidential information to develop school-to-post-school transition plans for young people leaving school before completing Year 12.
- Forging links with providers of employment services, community agencies, training organisations, and employers to create opportunities for young people to engage in programs that support successful transitions.
- Establishing and maintaining effective networks with school-based support staff across a cluster of schools to provide tailored support and interventions for individual case management.
- Utilising partnerships with business, industry, education and training providers, parents, and other stakeholders to facilitate casework support and work readiness decisions for individual student pathways.
- Delivering workshops, training, and preparatory programs to support students' readiness for post-school life.
- Providing Deaf awareness training to workplaces to support inclusive employment practices.
- Monitoring, reporting on, and evaluating student transition outcomes in line with departmental requirements.
About You
The following traits would be beneficial:
- Fluency in Auslan and a strong understanding of Deaf culture.
- Knowledge of youth mental health and wellbeing, particularly during transition periods.
- Experience in delivering training, workshops, or programs for young people and/or workplaces.
- Strong advocacy skills to support inclusive and student-centred transition planning.
- Well-developed communication, stakeholder engagement, and case-management skills.
- Proven ability to source, negotiate, and support work experience placements.
- Understanding of post-school supports, pathways, and relevant legislation.
- Demonstrated knowledge of evidence-based transition practices for Deaf and Hard of Hearing young people.
