Step Into Your Next Chapter:

• Supporting identified Year 6 students and their families in the transition to high school and, with securing referrals and engagement with external services to support student mental health prior to the end of the 2025 school year.
• Provide social emotional and mental health support to a key group of students and their families
• Providing small group lunchtime support for identified students
• Providing individual social/emotional mental health support, and running small group activities to provide wellbeing support.
