The Child & Family Health Nurse provides high-quality, evidence-based care to children aged 0–6 years and their families, supporting healthy growth and development within a primary health care model. The role involves assessing, planning, implementing and evaluating care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies and/or Australian Council of Midwives (ACM) guidelines, NSW Health and SESLHD policies.

You will undertake NSW Health child growth and development assessments; and maternal psychosocial assessments to identify family strengths and needs, provide health promotion, early childhood intervention, parenting support, and coordinate referrals and follow-up care. Collaboration with early childhood educators, schools, GPs and community partners is central to the role.

To be successful, you will be a Registered Nurse with current AHPRA registration and postgraduate qualifications in Child & Family Health Nursing. You’ll bring strong assessment, communication and coordination skills, and a passion for supporting family wellbeing and early childhood development.

Where you'll be working:

Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.

PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.

PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

The Role:

The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.

  • Child & Family Health Nurse, Children and Communities, coordinates and delivers high quality nursing care to clients of the Child Youth and Family Service consistent with SESLHD policies, procedures and standards. The Child & Family Health Nurse will have post-graduate qualifications in Child and Family Health to enable them to work within a primary health care model aimed at providing children aged 0 to 7 years with a comprehensive service.
  • The scope of Child & Family Health Nurse role is to: Assess, plan, organise, evaluate and deliver holistic quality nursing care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies and/or Australian Council of Midwives (ACM) guidelines, NSW Health and SESLHD policies and procedures, to achieve patient/client health outcome.
  • Provide health promotion activities and Child and Family Health Nursing clinical nursing services. This involves undertaking physical, emotional and psychosocial assessments to identify family's strengths and vulnerabilities to enable early identification and referral to appropriate interventions and social supports.
  • Work with early learning educators, principals, teaching staff, Learning and Support teams, school counsellors/ psychologists, general practice and local health and interagency agencies to deliver services to priority communitie
  • Contribute to care coordination of children and families by providing short term management, including active follow up for referrals

Selection Criteria:

  1. Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia, and relevant postgraduate qualifications related to the specialty area of Child and Family Health
  2. Demonstrated ability to work autonomously as well as work in a multidisciplinary (nursing, medical, allied health) and multiagency (health, education, early childhood services and non-government organisations) team environment
  3. Experience working in partnership with families delivering Child and Family Health Nursing Services within a primary health care setting
  4. Demonstrated experience in working with priority populations, specifically culturally and linguistically diverse communities.
  5. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
  6. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles.
  7. Basic computer skills and proven ability to use health information systems.
  8. Current unrestricted NSW driver's Licence with a willingness to travel in accordance with the demands of the position).
